Why is the male factor important in fertility treatment?
A couple aged between 29 and 33 with no fertility issues has about a 20–25% chance of conceiving in any given month. After six months of trying, 60% of couples will conceive without medical help. If after one year of unprotected intercourse there is no pregnancy, the couple is considered infertile. The male factor accounts for 34% of infertility cases, while 30% involve both partners. That’s why a thorough semen analysis can help identify potential issues and avoid costly and repeated Assisted Reproduction Treatments (ART).
A standard semen analysis only evaluates sperm concentration, motility, and morphology. This is incomplete, as it does not assess one of the most important parameters: DNA integrity. Also, understanding oxidative stress levels and sperm membrane quality allows for a more accurate diagnosis for reproductive treatment. Around 15% of infertile men have normal semen analysis results.
DNA fragmentation, oxidative stress, and poor sperm vitality are directly linked to lower fertility and embryonic development failures. This leads to more ART cycles required to achieve pregnancy—whether through IUI, IVF, or ICSI—lower embryo quality, and higher rates of miscarriage.
Depending on the levels of DNA fragmentation, oxidation, and sperm vitality, the medical professional will assess the semen sample and, together with the standard semen analysis, determine the best reproductive technique and treatment to improve the chances of success.
Antioxidant treatment can significantly reduce DNA fragmentation levels. The response depends on the root cause—if it’s due to toxic exposure or high temperatures triggering caspases and sperm endonucleases, it’s harder to modify sperm DNA fragmentation levels.
Sperm from the first fraction of the ejaculate typically has lower DNA fragmentation. Recurrent ejaculation or testicular sperm retrieval can also yield similar results depending on the patient. The effectiveness of these strategies should be evaluated by the medical professional.

Fragmentation above 30% impacts fertilization and embryo quality, leading to repeated miscarriages and lower reproductive success.

DNA Fragmentation (halosperm®): A non-invasive method analyzing ejaculated sperm. It distinguishes normal sperm (large chromatin dispersion halo) from damaged sperm (no halo).

DNA damage is multifactorial. Smoking, obesity, tight clothing, certain medications, fever, aging, chemotherapy, and radiation can increase fragmentation. Natural factors like improper maturation or oxidative stress also lead to damage during sperm production.

Freezing semen in liquid nitrogen does not affect DNA fragmentation. Tests can be performed on frozen samples, which can later be used in ART. After thawing, samples should be used quickly to prevent further damage.
